Consent language on the form
I agree to receive text messages from SubLine (Emnova Corp) about jobsite coordination for projects I work on. Message frequency varies, up to about 10 messages per week. Message and data rates may apply. Reply HELP for help, STOP to cancel at any time.
The checkbox is never pre-marked. Printed directly beneath it, on the same page:
- Messages are jobsite coordination only — schedules, deliveries, access, punch items. We never send marketing or promotional texts.
- Message frequency varies by job. Expect up to about 10 messages per week.
- Message and data rates may apply, depending on your mobile plan.
- Reply HELP for help. Reply STOP to cancel at any time.

This form is read, checked, and signed by the subcontractor who will receive the messages — not by the general contractor.

How the form is used
The general contractor presents the form during subcontractor onboarding at the jobsite trailer or with the project paperwork. The crew member reads the consent language, marks the checkbox, and signs. Only then is their mobile number added to the project in SubLine, and the first message they receive is the confirmation text described on the message flow sample page.
The general contractor retains the signed form as the record of consent for the life of the project and afterward. Any opt-out request — replied STOP, verbal, or written — is honored immediately. Questions: info@emnovacorp.com.
